This release form applies to all broadcasts produced by Brain Work Frame Work and/or Focused Business Solutions, LLC (Focused-Biz) (the “Company”) and interviewee (the “Guest” and/or “I”).

I authorize Brain Work Frame Work and/or Focused Business Solutions, LLC (Focused-Biz), it’s successors, and/or assigns to use the following personal or branded content; including but not limited to;

My Image – including photographic and electronic images/video

My Voice – including sound and video recordings

My Brand – including logo and other graphic images or likeness

Granted Uses:

I hereby  grant  the “Company”, its  subsidiaries,  licensees,  successors  and  assigns,  the  right  to use,  publish,  and  reproduce,  for  all  purposes,  my  name,  pictures  of  me in  film  or  electronic  (video)  form,  sound  and  video  recordings  of  my  voice,  and  printed  and  electronic  copy  of  the  information  described  in  sections  (1)  and  (2)  above  in  any  and  all  media  including,  without  limitation,  cable  and  broadcast  television  and  the  Internet,  and  for  exhibition,  distribution,  promotion,  advertising,  sale,  press  conferences,  meetings,  hearings,  educational  conferences  and  in  brochures  and  other  print  media.  Provided that the use of the “Guest” name and material(s) provided accurately reflects the nature of the interview. 

“Guest” reserves the right to request any misrepresented, distorted, or heavily edited material be immediately removed, if it does not accurately reflect the nature and purpose of the scheduled and recorded interview. This permission extends to all languages, media, formats and markets now known or hereafter devised.

This permission shall remain valid unless I revoke the permission in writing. If I revoke the permission in writing, the “Company” has 10 business days to remove any and all use of (1) my picture, and (2) my voice from all locations where it has been placed.​

I further grant the “Company” limited, non-exclusive, revocable use rights to my interest in the completed product; including but not limited to: live streaming replays, video recordings, audio recordings.

I give the “Company”, the limited, non-exclusive, revocable right to give or sell access to the final product, and exhibit the final product for purposes of marketing, communications, and or advertising purposes.
